From where can I request a W9 Form? As i cannot access help from the dashboard
You can submit a support ticket at https://dash.cloudflare.com/?to=/:account/support
This may be because of the current Cloudflare incident that’s related to the dashboard and API. I would suggest trying again once that incident is resolved

For me, due to recent Dashboard issues incident at Cloudflare Status Page, Page Rules are not loading (internal error 500) and sometimes I get Dashboard not working page.
