I pay royalties via local country based rights, but will Cloudflare block my live stream?

You can use Cloudflare Stream to deliver media that you have the right to use/distribute.
Cloudflare does not actively scan for and block copyrighted content like a platform such as YouTube might do.
If you only have the rights to use the media in a specific region, it is your responsibility to ensure users outside that region cannot access it.
Make sure the specific kind of content does not violate the Cloudflare ToS.
