Unable to send support request due to errors

I’m trying to send a new support request but keep getting these errors when clicking “Send”:

AFAICT, there are no such fields in the form. I’ve tried both Safari and Chrome.

I’m also getting the issue. It appears to be related to the select boxes on the form not storing the values selected. The “zones” and “category” fields are the issue category and domain select boxes, but the form isn’t detecting the selected values, that’s why the errors are coming up.

I was trying to submit a support ticket because this issue seems to be happening across the entire Cloudflare system.
