MP4s not playing on tvOS, Roku, Amazon Fire TV

What is the issue you’re encountering

We have had video channels on Apple TV, Roku, and Amazon Fire TV for years. Now, the videos aren’t playing on these channels, even though they work fine on all web browsers.
When trying to play on Apple, the error is “There is a problem loading this content.”
On Amazon Fire, the error is: “Video Playback Error: An unexpected error occurred.”
On Roku, videos that are less than 10 minutes will load, but anything longer will not play. They simply show a 99% load screen but never start.
I’ve tried using ‘yourdomain.com//.mp4*’ and bypassing the cache, which was suggested in old posts here. It didn’t help.
Any advice?

As I explained, the problem is on set top boxes like Roku, Apple TV, and Amazon Fire. There is no URL.
