Your screenshot shows they’re asking for both hostname and IP address.

Just look up the IP addresses associated with the two nameservers Cloudflare provided you. See: https://developers.cloudflare.com/dns/troubleshooting/faq/#where-can-i-find-my-cloudflare-nameservers

NB: If you get multiple IP addresses for either hostname, use any one of the IP addresses – doesn’t matter which one.

Also, note that you have to use the two different nameservers Cloudflare gave you. In your screenshot, you’ve entered the same nameserver twice.
