I have implemented cloudflare but still i am able to see exact server ip via via https://reverseip.domaintools.com/

Those are Cloudflare IPs. If you have PTR record published for a different IP points to your hostn,e and you wish to change that, you’d need to talk to whoever is responsible for maintaining that. Normally your hosting provider or network admin.
