Appears to be working as designed.
The photo was not AI created, it is genuine, I was forced to enter 5 DMCA Abuse reports, despite the instructions saying up to 250 URLs was possible in the menu entry box.
Secondly, I tried and tried for almost 20 minutes to find some way to contact Cloudflare to let them know what was happening to me, but finally my last resort was to post to general and post the photo as proof of what happened to me.

250 URLs are possible for the same host. The report will be forwarded to the hosting provider for that URL. Different hostnames or domains need their own report.
Could the text be improved before submission? Yes. But the error is clear. Were your 5 reports for the same FQDN? If not, it is working as designed.
Hello:
I did submit the best FQDN that I had at the time. Please understand that I did a ping back lookup, then a whois query, then a DuckDuckGo search query, then a website lookup from the DuckDuckGo results (I also queried Microsoft Bing and Google on the domain names)
Quite honestly, Cloudflare needs to do better.

The Cloudflare DMCA Abuse reporting website needs additional documentation, so that the average person will be aware that they need to file xxx reports for xxx websites. This was NOT clear from the reading of the DMCA Abuse webpage.
Let’s urge Cloudflare to improve.
