What steps have you taken to resolve the issue?

tried on other domains.
tried removing SSL(off) and other SSL modes
tried removing universal ssl and enabling it

Cf proxy off + ipv6 client = works
Cf proxy on + ipv6 client = doesn’t work

Cf proxy off + ipv4 client = doesn’t work
Cf proxy on + ipv4 client = doesn’t work
