Can't see Custom Rules in Load Balancer set up help article here says “On the Custom Rules page, select an existing rule or create a new rule.”
But I have no “Custom Rules” step in my create/edit Load Balancer workflow.

Are there any restrictions on using “Custom Rules” in Load Balancer set up?

I can’t speak to Enterprise plans, as they are custom configured, but even a basic load balancer should come with at least one custom rule. This is what I see when I set one up:

Eventually, “Custom Rules” section was enabled for my account. Via Cloudflare support ticket.
